Welcome to the Northwest Ohio Health Partnership (NWOHP) network website. Your employer has selected the NWOHP Choice PPO as your health plans network of preferred providers because the NWOHP Choice PPO network offers:

NWOHP Choice PPO is sponsored locally by Blanchard Valley Health System and the Hancock Medical Group physician association. Through our partnership with Ohio Health Choice, the NWOHP Choice PPO also includes a statewide network of health care providers. To look for a specific provider, or for a complete list of participating physicians, hospitals, and ancillary services please click on the link 'Physician Search', 'Facility Search' or 'Directory' buttons below. Please note that outpatient surgery facilities and centers in Hancock County include only the Blanchard Valley Hospital, Bluffton Hospital, and the Findlay Surgery Center (on Medical Blvd. in Findlay) as preferred 'in-network' providers.
